If you are a parent or guardian of a Wisner-Pilger student that will be playing sports this school year, and you were unable to attend the annual Sports Warning Meeting, please watch the video recording of a previous meeting and sign the Sport Warning Meeting (Video) Form. Attending the meeting (or watching the video recording) and signing the corresponding form is a requirement to participate in sports at Wisner-Pilger.

Reminder: All 8th-12th grade students that plan on participating in sports and/or NSAA sponsored activities, and all incoming 7th grade students (regardless of sports participation) must have a Pre-Participation Physical completed.
- 8th-12th graders must have physicals completed before the first day of practice.
- 7th grade students must have physicals completed before the first day of school.
Pre-Participation Physical Forms are available in the Jr/Sr High School Office, or can be found at this link: http://5il.co/3iw8 (En Espanol: https://5il.co/gwkv). Medical Providers may use their own form, but the last page of the linked form must be completed regardless of which form is used.
The best approach to completing physicals is to use the form provided in this article, complete the physical during June or July, and then submit the completed form to the Jr/Sr High School Office.
